Where do I get an application form?

If you are resident outside of Ireland, you need to use an APS2 application form.

APS2 forms are available from the Embassy, Kazanches, Guinea Conakry Street, Addis Ababa (location directly behind the Radisson Blue Hotel) during consular services opening hours:

8.30am - 12.30pm & 1.30pm - 4.00pm Monday to Thursday,

8.30am - 11.00am on Friday.
